Trauma Registry Data Dictionary

2026 Data Dictionary
The state of Ohio data dictionary now incorporates an Ohio Regional Data Dictionary that includes regional specific fields, starting on page 271.
COTS members have opted not to collect the following data elements from the Ohio Regional Data Dictionary: 

  • Cause Code, pg 272 (will continue to collect ICD-10 Code)

  • Injury Details, pg 273 (will continue to collect ICD-10 Code)

  • Hospital Procedure Code, pg 278 (will continue to collect ICD-10 Code)

2025 Data Dictionary
The state of Ohio data dictionary now incorporates an Ohio Regional Data Dictionary that includes regional specific fields, starting on page 268.
COTS members have opted not to collect the following data elements from the Ohio Regional Data Dictionary: 

  • Cause Code, pg 269 (will continue to collect ICD-10 Code)

  • Injury Details, pg 270 (will continue to collect ICD-10 Code)

  • Hospital Procedure Code, pg 276 (will continue to collect ICD-10 Code)

    • Injury Details, pg 4 (will continue to collect ICD-10 Code)

    • Hospital Procedure Code, pg 10 (will continue to collect ICD-10 Code)
